Rates for 2006-07 school year:
$140 for 4 lessons ($35 per hour)
$175 for 5 lessons

• Students are asked to pay the month’s lessons in advance (at the first lesson of the month).
• Cash or Checks accepted
• Checks for lessons should be made out to: Amy Roark-Oblak
• Return Check Fee: $25 per occurrence plus any expenses incurred

If a student misses a lesson without 24 hours advanced notice of the anticipated absence, the lesson will not be made up and no refund given. Double scheduling of activities (church groups, after-school music groups, other pre-scheduled professional appointments, etc.) will not be excused, as these are avoidable conflicts and advanced notice of the conflict can be given and the lesson rescheduled.

If a student misses a lesson with 24 hours advanced notice of the anticipated absence, an opportunity to makeup the lesson will be given within the same month. It is the parent/student’s responsibility to call and schedule this make up lesson.

Same-day cancellations, by student, will not be made up for any reason.

Make up lessons will be limited to two per student per quarter. Beyond this limit, canceled lessons will not be made up nor refunds given. (This does not include potential makeup lessons due to teacher cancellation):

• Fall: September, October, November
• Winter: December, January, February
• Spring: March, April, May
• Summer: June, July, August

Since I draw students from several school districts or cities, it is virtually impossible to designate a specific week as 'Spring Break'. Therefore, students may continue their studies during their appointed Spring Break week, or they may choose to make up the lesson later in the month. If the latter is chosen, the missed lesson counts as one of the two allowed misses per term.

I will schedule a makeup lesson in the event I need to cancel a lesson. Advance notice will be given, unless sickness, injury or family emergency occurs. Lessons falling on holidays will be made up at the discretion of the teacher, and will depend on availability. If lessons are cancelled due to inclement weather, the month’s lesson payment will be advanced one week.

Students must be on time for lessons and should allow ten minutes after a lesson in case the teacher is running behind schedule.

If lessons are being discontinued, a 15-day notice is requested.
